To access our website you must have an authenticated account. This account can only be used for one person, and will contain all of your order information. You will also be able to fill out and submit your medical history online through our secured, HIPAA compliant servers.
3-Step Process
1.) Create your account by purchasing a product on our store.
2.) During your checkout process, your account will be created and you will be logged in. If you need to log in, you can click here. Once logged in, you can fill out medical history questionnaire here. You must be logged in with your account to access this page. This must be done within 48 hours of your purchase.
3.) Once you have filled out your medical history, our provider will contact you to set up your appointment for your consultation.
